RESUMEN

Creating notes in the EHR is one of the most problematic aspects for health professionals. The main challenges are the time spent on this task and the quality of the records. Automatic speech recognition technologies aim to facilitate clinical documentation for users, optimizing their workflow. In our hospital, we internally developed an automatic speech recognition system (ASR) to record progress notes in a mobile EHR. The objective of this article is to describe the pilot study carried out to evaluate the implementation of ASR to record progress notes in a mobile EHR application. As a result, the specialty that used ASR the most was Home Medicine. The lack of access to a computer at the time of care and the need to perform short and fast evolutions were the main reasons for users to use the system.

Breast cancer represents 23% of all cancers diagnosed among women each year. BRCA1 and BRCA2 are tumor suppressor genes related to the most frequent form of hereditary breast and ovarian cancer, as well as other types of cancer. The aim of this work is to describe the development of Clinical Decision Support Systems (CDSS) for referral to genetic counseling in patients at increased risk of pathogenic variants in BRCA1 and BRCA2, and to describe results during the pilot study implementation (from January 5, 2021 to March 5, 2021). To achieve integration and system interoperability, we used FHIR and CDS-Hooks within the CDSS development. A total of 142 alerts were triggered by the system for 72 physicians in 98 patients. Results showed an acceptance rate for the recommendation of 2.1%, which could improve using intrusive alerts in all of the hooks.

The high prevalence of PIMs in elderly is a major healthcare concern and indicates the need for medication monitoring systems. Most PIM CDSS have shown positive effects respecting PIM prescription but these results were more consistently in hospital settings compared with ambulatory care. We describe the post-implementation evaluation of a PIM CDSS for general practitioners (GP) in the ambulatory setting and explore GP interactions with the PIM alerts. The CDSS generated 3218 unique alerts and involved 2863 elderly patients. Benzodiazepines was the drug with the most alerts triggered. Only 129 (4 %) were opened by GP during patient appointments. We need to develop an understanding of how alerts should be designed and display information to support the workflow of general practitioners. Pos-implementation evaluations are the key of CDSS improvements.

30% of the Argentinian population and 58% of Plan de Salud HIBA patients are unaware of their HIV status. The Ministry of Health and US Preventive Service Task recommends physicians to assess HIV infection in persons aged 15 to 65. An HIV screening reminder integrated in an electronic health record (EHR) was created using FHIR to represent clinical information and CDS-Hooks to represent the exchange of information with a CDS service. The tool had a 1% intervention rate, and 67.4% acceptance rate. The number of HIV screening tests requested during the weeks after the CDSS implementation and in the same period in 2017 were obtained. 575 orders were requested in the 2017 period and 893 in the 2018. 89 (almost 10%) of these came from the electronic tool. The preliminary results indicate that this non disruptive, action oriented reminder can contribute to increased HIV screening orders.

RESUMEN

An accurate and updated problems' list is critical in a problem-oriented Electronic Health Record (EHR). The lack of organization and maintenance of the problems limits its value. Certain problems have a larger effect on the clinical evolution of the patient, these are known as Comorbidities. The aim of this paper is to evaluate the impact of the automatic loading of comorbidities in the organization and maintenance of inpatient problems' list using a comorbidities subset.

Los errores en la medicación son comunes en la práctica médica, más aún en los hospitales universitarios. Aquellos asociados con la dosis de los fármacos son los más frecuentes. Los sistemas computarizados de prescripción médica (CPOE, sigla en inglés para Computerized provider order entry) han demostrado mejorar la tasa de error e incrementar la calidad de cuidado médico. Nuestro hospital desarrolló su propia Historia Clínica Electrónica con un completo CPOE. Los farmacéuticos validan todas las prescripciones y rechazan aquellas que contienen errores, siendo los más frecuentes los vinculados conerrores en la dosis. Un grupo multidisciplinario acordó una lista de posología recomendada de fármacos que se ofrece a los médicos tratantes luego de que seleccionan la droga a prescribir. El objetivo de este estudio es describir el desarrollo e implementación de una estrategia de prevención de error que incorpora dosis recomendada en el proceso de prescripción electrónica.


Os erros de medicação são comuns na prática médica, ainda mais em hospitais universitários. Aqueles associados com a dose das drogas são as mais freqüentemente. Os sistemas informatizados de prescrição médica (CPOE,sigla em inglês para entrada de pedido do provedor informatizado) foram mostrados para melhorar a taxa de erro e aumentar a qualidade dos cuidados médicos. Nosso hospital desenvolveu a sua própria registros médicos eletrônicos com uma completa CPOE. Farmacêuticos valida todas as prescrições e rejeitar aqueles que contêm erros, sendo os mais freqüentes aqueles relacionados com erros na dose. Um grupo de trabalho multidisciplinar chegou a acordo sobre uma lista de dose recomendada de drogas que é oferecido aos médicos que depois que selecionar a droga de prescrever.


Medication errors are common in medical practice, even more in teaching hospitals. The errors associated with drugs doses are the most common. Computerized provider order entry (CPOE) has demonstrated that could improvethe error rate and increase the quality of medical care. Our hospital developed its own Electronic Health Record(EHR) with a full implemented CPOE. Pharmacists validate all the prescriptions, and reject those that contain errors,the most frequent errors are related to dose. A multidisciplinary group agreed on a list of recommended dose of drugs, that is offered to the treating physicians after selecting the drugs to prescribe. The aim of this study is to describe the development and implementation of a prevention strategy that incorporates recommended error in the process of electronic prescribing doses.
